CANCELLATION POLICY

We understand that unanticipated events happen occasionally in everyone’s life. In our desire to be effective and fair to all patients, the following policies are honored:

48-hour advance notice is required when cancelling an appointment. This allows the opportunity for someone else to schedule an appointment. If you are unable to give us 48-hours advance notice, you will be charged $25 per hour of your scheduled appointment. (For example, $50 would be charged if you had a 2-hour appointment scheduled.)  This amount must be paid prior to your next scheduled appointment.

72 -hour advance notice is required when canceling a scheduled surgery. If you are unable to give us 72-hours advance notice, you will be charged $100.  This amount must be paid prior to scheduling future surgeries.

LATE ARRIVALS

If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, we will determine if there is enough time remaining to start your session. Regardless of the length of the session actually given, you will be responsible for the “full” session.

IF WE CANCEL

If we cancel your appointment without 48 hours notice because of a clinician illness, we will register a $25 credit per hour of scheduled appointment on your account
